OBJECTIVES :

(a)  To support and promote a healthy populace that is vital to the development and progress of the city by providing them access to free medicines; and

(b) To ensure inclusiveness, encourage civil contribution and participation and dispel the counter-productive effects of discrimination by offering free essential medicines to all qualified beneficiaries regardless of status in life.

 

BENEFICIARIES :

(1) Resident :
     a.  An actual resident for one (1) year as certified by the Punong Barangay in the barangay
          where he is residing; and
     b. A registered voter as certified by the Comelec. 

(2) Non-Resident : A registered voter of the City of Borongan who has at least voted for the last two
      (2) elections, except barangay elections as certified by the Comelec. 

(3) A dependent of any of the above.

 

DEPENDENTS :

1.  Minor:
     a.  A legitimate, legitimated, illegitimate or legally adopted child of the beneficiary;
     b.  Chiefly dependent upon and living with the beneficiary; and
     c.   Not more than 18 years old, unmarried and not gainfully employed.

2.  Adults (18 years old and above):
     a.  Any person who is incapable of self-support because of mental or physical inadequacy under
          the care of the beneficiaries; and
     b.  Is a registered voter of the City of Borongan as certified by the City Office of the COMELEC
          when applicable.

 

HOW TO AVAIL FREE MEDICINE :

A. If beneficiary will directly avail:
    (1)  Free Medicines ID card;
    (2)  Doctor’s prescription;
        a. Maintenance medicine – must be issued within the last three (3) months reckoned from the
            date of availment;
        b. Antibiotics – must be issued within seven (7) days reckoned from date of availment.
        c.  In all cases, the prescription must indicate the date and the name of the patient; and it
             must be duly signed by the attending physician with his/her license number and contact
             details; and

      (3)  Free medicine booklet.

B.  If through a representative, provided that the beneficiary is either physically or mentally incapacitated to personally avail of the free medicines:
    (1)  Authorization letter duly signed by the beneficiary;
    (2)  Valid ID of the representative;
    (3)  Valid ID of the qualified beneficiary; and
    (4)  All the other requirements enumerated in (a.).

 

REQUIREMENTS FOR THE AVAILMENT OF FREE MEDICINES IDENTIFICATION CARD (ID CARD)
A Free Medicine ID card shall issued to a beneficiary, indicating therein his or her dependents. The City Social Welfare and Development Office (CSWDO) shall issue the Free Medicines ID Card. The requirements which shall be strictly enforced for the issuance of the said card shall be the following:

   a.  For a resident beneficiary, original or certified photocopy of the following documents: 
        (1)  Duly filled out and notarized application form;   
        (2)  At least one (1) valid government-issued ID;
        (3)  Certificate of residency from the barangay which must have been issued within six (6)
               months reckoned from the date of   application stating therein actual residency for at
               least one (1) year; and
        (4)  Comelec Certification which must have been issued within one (1) year reckoned from the
              date of application stating therein that the person is an active and registered voter of the
             City of Borongan.

   b.  For a non-resident beneficiary, original or certified photocopy of the following documents:
       (1)  Duly filled out and notarized application form;
       (2)  At least one (1) valid government-issued ID;
       (3)  Comelec Certification which must have been issued within one (1) year reckoned from the
             date of application, stating therein that the person is an active and registered voter in the
             City of Borongan and has actually voted during the last two (2) elections, except barangay
             elections.

   c.  For the inclusion of dependent/s in the list of dependents of the beneficiary:
      (1)  For minor dependents: original or certified photocopy of the birth certificate issued by
             the Philippine Statistics Authority (PSA) or the City Civil Registry.
      (2)  For persons with disabilities and senior citizens: photocopy of the PWD or Senior Citizen ID.

 

PENALTY CLAUSE
The following violations shall be meted with the corresponding penalties in accordance with pertinent laws and issuances :

     a.  Any misrepresentation made by a beneficiary, dependent or his or her representative to
          avail of the free medicines:    
         (1)  First Offense. Stern warning.
         (2)  Second Offense. 6 to 12 months suspension in availment of free medicines depending on
                the discretion of the CBFMB.
         (3)  Third Offense. Perpetual disqualification to avail of the free medicines and/or
                imprisonment of thirty (30) days to one (1) year.

     b.  Any misrepresentation or violation of this ordinance made by any national, city or barangay
           government official or employee shall be subjected to civil, criminal and administrative
           liabilities.
